What You Need to Know

You and your dependents (up to age 26) are covered under the life and accidental death and dismemberment insurance coverage plan if your collective bargaining agreement provides this benefit.

Coverage is administered by Voya Life Insurance Company.

Life Insurance

Life insurance pays a benefit to your beneficiary when you pass away. The amount depends on the option or class that your employer has negotiated. If eligible, you may also enroll your dependents.

If there is a death, beneficiaries should contact the Los Angeles Fringe Benefit Office as soon as possible to begin the life insurance claim.

Accidental death and dismemberment insurance (AD&D)

The accidental death and dismemberment (AD&D) insurance provides a benefit to you if you lose your life, limb, or sight due to an accident. If you die from a covered accident, your beneficiary will receive a benefit in addition to your life insurance benefit.

The following conditions must be met:

  • You are insured on the date of the accident.
  • The loss occurs within 180 days after the accident.
  • The cause of the loss is not excluded from the policy.

The payment for all losses caused by any one accident will never be more than the 100% of your life insurance amount (which depends on the option/class that your contributing employer has negotiated for).

Don’t forget to designate a beneficiary!

It’s important to have a beneficiary on file and keep it up to date when you marry, divorce, have a child, or experience any other life event that might influence whom you name as beneficiary. You may change your beneficiary at any time by submitting a new beneficiary form to the Fund Office.
